This project proposal provides for the provision of quality personal mobile integrated health and social services in a home environment for people with disabilities, people over 65 years of age unable to self-care and adults at risk living on the territory of Cherven bryag municipality. The planned activities will be provided in an innovative way in order to provide personal health, social and psychological assistance and support, counseling services for prevention and healthy living, etc., which will contribute to improving access to new integrated services for people in need and overcoming social exclusion while at the same time building on the social services currently available in the community. A major focus will be on the adequate provision of quality comprehensive health and social care and support at home to people living in remote areas where access to these services is severely restricted or lacking. It is envisaged to reach 140 users within the Cherven bryag municipality for a period of 18 months. The project activities are aimed at building a sustainable model for quality provision of social services on the territory of the municipality and are organized in a way that guarantees optimal use of human and financial resources.
