Active support for the dignified life of people with disabilities and special needs in Hadjidimovo municipality

This project proposal aims to improve the quality of life and encourage the social inclusion of disadvantaged people and those with special needs on the territory of the Hadzhidimovo Municipality by providing access to social inclusion and healthcare services. The specific goal of the project is to improve the access of people with disabilities and elderly people aged over 65, with limitations or inability for self-service to social inclusion services in the community and in the home environment. The target group includes 24 participants, representatives of the following vulnerable groups – people with disabilities and elderly people over 65, with limitations or inability for self-service. The project foresees to expand the activity of the municipal Social Service Center, created procedure "New Opportunities" under the HRD OP 2014-2020, by employing the necessary staff to provide the planned services (social worker, medical specialist, personal assistants and home assistants, psychological support specialist), organizational and material provisioning of the activities. Increasing the capacity of the Center will allow for a minimum of 24 users to be helped and supported for a fulfilling existence, by providing complex hourly intersectoral services (social-health, psychological, motivational, etc.) which include care for satisfying daily needs, support in the execution of various community activities, social work and consulting aimed at fulfilling the needs for leisure time organization, creating contacts, etc. People from the target group will have the opportunity to choose the service they want and require according to their specific needs, based on an individual evaluation of their needs. As a result of completion of the project activities, users will be provided access to various services in support of their independent and dignified life, as well as for their social inclusion and integration within the community.
