Support for social inclusion of people with disabilities and lonely adults in the municipality of Garmen

The main objective of the project is to improve the quality of life, promote social inclusion and integrate into society disabled people and people over 65 not able to self-service themselves, on the territory of Garmen Municipality, by providing access to social and health services in home environment and in the community. Specific objectives: - To extend the activity of the Municipal Center for hourly provision of social services to disadvantaged people, established under the “Independent Living” Operation of HRD OP 2014-2020. - Improve access for disabled people and lonely living elderly ones to high quality integrated social and health services in the community and in their home environment, tailored to their individual support needs. The project includes 24 persons, representing the following target groups: - disabled people; - people over 65 not able to self-service themselves. The project objectives will be achieved through the following main activities: - Provision of supportive services, incl. integrated cross-sectoral services in the community or in a home environment for the elderly (including lonely people over 65 not able to self-service themselves) and for people with disabilities. - Improving access to health and health promotion, incl. through specific actions targeted at people with disabilities, people over 65, not able to self-service themselves and adults at risk. As a result of the implementation of those activities, the scope of services, provided in the Municipality of Garmen for social inclusion of persons from vulnerable groups, will be extended. Providing access for people with disabilities and elderly ones over 65, unable to self-service, to sustainable integrated social-health services based on their individual support needs will contribute to raising their quality of life and will have a preventative role regarding the risk of them falling in poverty and social exclusion.
