"For a better life in Sredets municipality"

The proposed project is aimed at improving the quality of life and access to social inclusion services, including health, in response to the complex needs of disabled people, people over 65 in self-care and elderly people at risk from the Sredets municipality, overcoming social exclusion and poverty. The provision of quality integrated services in the community and in the home environment is a mechanism for preventing the risk of falling into an institution. At the same time, conditions for acquiring skills for independent living, taking into account the physical and social status of the users, are created. The project will expand the activities of the Center for Social and Health Services - CSZU, established under the HRD OP "Independent Living" scheme. The project fully overlaps the objectives of the MIG MEDEDIA - BETTER LIFE OF THE TERRITORY scheme and will respond to the need to provide assistance to target groups to organize their own lifestyle and daily activities as well as the lack of social contacts. The provision of hourly services within the CSDP will be staffed to serve the people in need. The CSA team will include 16 persons.The provision of services will be based on the identified needs and problems, and the individual social assessment of the target group's representatives in need of constant care and inability to self-handle. We plan the project to serve 50 people. The service - personal assistant will be provided in the homes of the users. For a better quality of service provision, we expect the complex of social, health, psychological and rehabilitation services to be provided on an hourly basis, both in the CSA and in the homes of the consumers.The Municipality of Sredets has a specialized car with a ramp for disabled people, purchased with funds provided by the Independent Living Scheme of the HRD OP, which will take the users from their homes to the CSD and vice versa.
