Social Innovations for Active Social Inclusion in Pavlikeni Municipality.

The project proposal includes activities by which the beneficiary will provide the social service “Home Helper” and the health service “Medical Rehabilitation”. The users of the services wil be supported by a consultant who will carry out discourses, make recommendations and consultations on the up-to-date topics for the target groups. The assistant will make materials about the principles of healthy life and problems connected to the health condition of the vulnerable groups, and the project organization and management team will distribute them among the population and the target groups. For this purpose the persons who will use the social service will be selected; selection procedure will be held among the people wishing to work to the position “Home Caregiver”. A person will be appointed to the position “Social Activities Assistant” and he/she will coordinate the work of the staff with the users. The innovative moment which will improve the users’ access to health services in the settlements of the municipality, incl. the remote ones, consists in providing medical rehabilitation in the users' homes. Up to now such a healthcare service has not been provided on the territory of the municipality. Therefore a rehabilitation therapist will be appointed. Target groups: 1. People with disabilities and their families; 2. People over 65 years of age who are unable to self-care. Taking into consideration the achievement of the project goals the activities planned for implementing are: 1. Selection and appointment of the project staff; 2. Provision of supporting social service “Home Helper”; 3. Provision of health service “Medical Rehabilitation” and healthcare via target actions; 4. Provision of information for the population about the principles of healthy life and health problems. The project implementation will significantly improve the access to healthcare and social services of people from the target group from all the settlements in the municipality.
