Social innovations for active social inclusion in Polski Trambesh municipality

The project proposal envisages upgrading of the completed project “Independent life in Polski Trambesh municipality”, OP “HRD” 2014-2020. At the beginning of the project the persons wishing to use the social service will be identified and selected, as well as a selection of persons wishing to work as a home caregiver shall be made. The Center for Social Activities will be set up for the project's needs. One person will be appointed to the position of Organizer of Activities, who will coordinate the work of the staff with the users. An "Assistant, Social Activities" will also be appointed. The innovative element that will improve consumer access to health services in the various settlements of the municipality, including remote places, is to provide advice on healthy lifestyles and nutrition. about the opportunities and the need for medical rehabilitation of consumers and at the Center for Social Activities. To date, this type of health service has not been provided on the territory of the municipality. A medical specialist will be appointed for this purpose. The healthcare professional will develop and provide consumers with healthy lifestyle and nutrition materials. Basic activities: 1. Selection and appointment of staff for the provision of the social service 2. Provision of support services. 3. Provision of health care activities to the target groups through consultation with a healthcare professional. Target groups: 1. People with disabilities and their families; 2.People over 65 with no self-care. As a result of the project implementation it is expected: 1. Improve access of people with disabilities and people with disabilities to self-care from all settlements on the territory of the municipality, including hard-to-reach places for social services and health counseling. 2. To improve the quality of life of the target groups; 3. To create prerequisites for improving the health status of the persons covered.
