HOUSEHOLD INTEGRATED SERVICES FOR THE LOCAL COMMUNITY OF THE VILLAGE OF BOLYARSKO

This project proposal will provide and guarantee an effective and efficient model of implementation and delivery of social and health services, fully tailored to the specific conditions and needs of the local community. The project will build on the experience and good practices in the village of Bolyarsko in the delivery of social services in the community Home social patronage and Public Dining Room, complementing the impact of these services with full integrated home health and social care and other active measures for social inclusion and independent living, for both potential users and their families. At the same time, this approach and the set of integrated measures for social inclusion will practically be the safest and most complete prevention of institutional care and inclusion in social services in the residential community, which necessarily implies the removal of the consumer from the family environment. The wide range of integrated health - social interventions with rehabilitation, psychological and community support is aimed at direct work not only with the user but with the family members, which is undoubtedly the most correct, effective, efficient approach. The innovative solutions for integrated home care services such as the inclusion of personal care, medical care, psychological support, rehabilitation interventions, active social work and family support activities for disabled people from the local community of Bolyarsko village, municipality Tundzha will greatly offset the deficits of the environment and increase the well-being of the local community and orator LAG "Tundzha".
