"Shared Horizons" - support for elderly people at risk and their families in small rural settlements

The project "Shared Horizons" – support for elderly people at risk and their families in small rural settlements " is an innovative, integrated complex multi-component concept to support local communities in small settlements of rural type and in Territory of the LAG – Tundzha. The project includes delivery of innovative integrated health – social services and other supportive and socializing activities for at least 45 adults at risk, including people with disabilities and their families in three of the settlements in the territory of LAG – Tundzha. The range of consumer services includes: home care; health and social consultation, and involvement in socialized community initiatives for active inclusion. The project concept is closely linked not only to direct interventions by a multidisciplinary team with regard to improving the quality of life of potential users, but also to their relatives / families. The goals and activities are fully tailored to the needs of the local communities and are related to the environmental deficits in small rural areas and are related to the expectations of potential target groups. The applicant is a licensed home service provider. The proposed approach in the project is innovative, also because it involves a real multidisciplinary model of decentralization in the delivery of integrated health and social services in small rural settlements by the state and local authorities to other alternative providers.The present project concept is innovative and fully compliant with the specifics of the LAG - Tundzha territory. The implementation of the planned activities by applying an innovative model will increase and upgrade the range of services for vulnerable communities in small rural settlements.
