A new center for social inclusion and development, social rehabilitation and community integration

The current project proposal includes providing of access to innovative cross-sectoral services to adults at risk, according to their specific needs at a newly established Center for Social Inclusion and Development, Social Rehabilitation and Community Integration in Rakovski municipality. The Center is a separate unit that will support the individual development, promoting and acquiring of new capabilities, and self-dependency of 50 persons with different types and degrees of disability, as well as support for adults at risk through targeted activities for integration and prevention of social exclusion. The service includes a variety of counseling, therapeutic and rehabilitation activities, aimed at creating, supporting and expanding the social skills, training for self-living, various consulting activities related to social assistance, social and health services, assistance in filing documents for different institutions and others. The Center will also work to improve access to healthcare, health promotion and social inclusion, through health counseling, and local social activities, by creating conditions for social contacts, various manifestations, visits, excursions, etc. The Center’s team will include specialists with a different profile of competence - social, medical, health, pedagogical and other professional qualifications. Each of the specialists will be trained to work with the target group. The people use activities (individually or in a group) hourly - every day or several times a week / month depending on their specificl needs. The target groups of the project are: 1. Adults at risk 2. People with disabilities 3. Old people over 65, unable to self-service, who are on the territory of all settlements in Rakovski municipality.
