New long-term care for the elderly and people with disabilities - provision of new services

This project proposal is aimed at providing support to the process of deinstitutionalization of care for the elderly and people with disabilities through the development of community services, providing them with an independent and dignified life and full inclusion in society. Its implementation is in line with the Action Plan 2018 - 2021 for the implementation of the National Strategy for Long-Term Care and is part of the overall process of deinstitutionalization of care for people with disabilities and the elderly and building alternative community support by providing quality and affordable long-term care services. During the implementation of the project, two social services will be opened and provided for a period of 12 months - Care Center for People with Mental Disorders and Care Center for Elderly People incapable of Self-Care, for the following target groups: 1. Elderly people over 65 with inability to self-care from the community; 2. Persons with mentaly disorderss and their families. 4. Employees of social service providers. For the needs of the Centers, additional equipment and furniture will be purchased for the premises where the new services will be provided. To ensure quality and effective social services, staff will be selected and hired and will undergo training and supervision. In order to ensure a multidisciplinary approach and quality of service provision, rules of operation and requirements governing the operation of the new services will be developed. In order to ensure transparency in the implementation of the project activities and to promote the objectives and activities of the European Union, the European Social Fund, the HRD OP and the current project proposal, information and publicity activities will be carried out. The duration of the project will cover a period of 18 months.
