Support for social inclusion of people with disabilities and elderly living alone in Roman Municipality

The project proposal aims at improving the quality of life and overcoming the consequences of poverty and social exclusion among people with disabilities and people aged over 65 in Roman Municipality through improvement of the access to integrated services for social inclusion - health and social community-based services. In order to achieve this objective we plan to upgrade and further develop our Center for hourly provision of community and home-based services for social inclusion which has been established under the "Independent Life" grant award procedure. The center provides tailor-made integrated services to the representatives of the target groups based on individual plans. In order to widen the volume, the variety and scope of the services provided a team of experts will be established comprising of a nurse, a health mediator, domestic assistants, a psychologist and a coordinator of the center - altogether 9 persons employed on labour contracts. Within the center the clients will have access to social, administrative and medical services, will be able to socialize and to have access to up-to-date information and facilitation of the communication with various institutions. The domestic assistant service will be provided at home. All clients will undergo a individual necessity assessment in order to guarantee that all clients will receive the service or the set of services they actually require. As a result from the implementation of the activities the scope of the provided social inclusion services to persons from the vulnerable groups in Roman Municipality will be widened. Provision of access to integrated social and health services based on individual requirements to people with disabilities and elderly aged over 65 who are not able to take care of themselves will lead to increase in their quality of living and will prevent the risk of poverty and social exclusion. At least 30 people from the target groups will be included in the project.
