Integrated social and health services in Lukovit municipality

The project plans the renewal and extension of the activities carried out in the Center for hourly services for social inclusion in the community or the home environment.The main project activities implemented through the Centre include: support services for elderly over 65 years of age who are unable to self-care and for people with disabilities to meet their daily needs at home,improving the access to healthcare,promoting the health and delivering of local social activities for social inclusion.The center will also provide mobile integrated social health services in the smaller settlements in the municipality.A team of part-time workers will be assigned to the Center:Coordinator,18 home assistants,a nurse,a rehabilitator,a psychologistsocial worker/occupational therapist and a driver.Minimum 40 clients will be using the services, all of them will go through a social assessment in order to meet their individual needs.In addition to the individual approach,there will also be an integrated approach for the provision of social and health services, health prevention services, social inclusion services to overcome social exclusion and lack of social contacts for people over 65 and people with disabilities - providing psychological and/or social rehabilitation according to the individual needs of the person.The clients, with the support of the team,will be able to choose the services they need the most.There will also be activities targeted to the local community aiming at overcoming negative stereotypes toward people with disabilities. The project completes the municipal strategy for social services with the implementation of local social inclusion activities. In these activities,on the one hand,public gatherings on occasions and organized lectures will be conducted, and on the other–products made by the clients will be offered at charity bazaars,and the service users will not only perceive the public's concern for them,but also the fact that they are useful to society.
