The aim of the project is to improve the quality of life and the opportunities for social inclusion of people with disabilities and the elderly in the Municipality by providing a network of services in the home environment and building adequate material and personnel capacity for their provision by introducing a model for patronage care for elderly people and people with disabilities, incl. with chronic illnesses and lasting disabilities to provide hourly mobile integrated health and social services in their homes.
