Patronage Care+ in Municipality Novi Pazar

The project proposal provides for the provision of integrated social and health services for people with disabilities and at risk, in connection with limiting the spread and overcoming the consequences of COVID-19. The project proposal will provide an opportunity to ensure a network of services in the home environment and will ensure the provision of hourly mobile health and social services in homes for people with disabilities and the elderly incapable of self-care. According to the indicative number of users determined for each municipality for inclusion in the project, it is envisaged that at least 54 persons will use the integrated social and health service. The operation will be carried out by applying the already established model and unified programs for providing hourly mobile integrated health and social services and staff training. The implementation of the project activities will ensure higher efficiency of the provided services using significantly small financial resources, by applying flexible mechanisms for combined hourly use of integrated social and health services, oriented to the individual needs of each needy. This project will improve the quality of life and access to social inclusion services in response to complex health needs, including those of people with disabilities and people who are unable to self-care, in order to overcome the effects of social exclusion. For the optimal functioning of the service, a room for the service management team and the specialists will be repaired and furnished and equipped. In order to expand the scope of social and health services on the territory of the whole municipality and to ensure equal access of the persons to them, a vehicle will be purchased with which the specialists in social and health care will be able to reach each of the users.
