Patronage care + in the municipality of Sandanski

"Patronage Care +" gives the opportunity to provide hourly social and health services in the home environment to elderly people over 54 years of age who are unable to self-care, people with disabilities or quarantined people due to COVID-19. They can rely on Patronage Care + teams to deliver food, medicines and basic necessities purchased with their own funds, as well as to assist in urgent administrative services. The specific objective of this project proposal is to develop a model for patronage care for the elderly and persons with disabilities, incl. with chronic illnesses and permanent disabilities to provide hourly mobile integrated health and social services in their homes. A model for patronage care for the elderly and the disabled, including chronic illnesses and permanent disabilities, will be built to provide hourly mobile integrated health and social services in their homes. The project activities will be aimed at elderly people over 54 years of age, with limited or inability to self-service, as well as to people with disabilities living in remote and depopulated settlements in the municipality of Sandanski, which limits their access to health and social services. Individuals will receive hourly specialized medical services, and for the elderly with chronic illness and permanent disability, specialized medical services will be provided in their homes. The specific objective of direction 2 is to provide personal protective equipment, PCR and rapid antigen tests and disinfection to ensure the effective functioning of social services in the Municipality of Sandanski, as well as ensuring the safety of their employees and users.
